- Complete online application form at http://passport.gov.in/nri/Online.do
- Upload child's recent passport size colour photograph on passport portal;
- Download the online application form and print it out;
- Fill up the printed application form's all blank columns in capital letters;
- Paste child's recent passport size colour photograph on the application form;
- Sign the form child & parents at the specified places;
- Download Personal Particulars Form (PP Form) from https://www.hciwellington.gov.in/docs/PPForm.pdf;
- Download Annexure - E form from https://www.hciwellington.gov.in/docs/Annexure-E.pdf;
- Download Annexure "D" Form;
- Fill up & Parents must sign PP Form & Annexure-E & D Forms;
